The scope is to empower the participation of all Omya employees in incident notification through a platform by defining the existing or possible nonconformities, incidents and risks, eliminating root causes and prevent occurring of them.

Incident: It is the umbrella term for accident and near-miss. An Incident is an unplanned event or occurrence that results in unwanted outcome or has the potential to negatively affect: people, property/asset/equipment, environment, image, quality, stakeholders. Too often, the natural human tendency is to ignore the incident: nobody was hurt, no expense incurred, too trivial to act altogether. But ignoring the incident, we may be losing a valuable lesson, failing to notice a more serious problem lurking beneath the surface, ignoring a systematic tendency that one day will create a major malfunction.
Along with health programs, safety education, H&S risk assessments and regular safety inspections, OMYA created the Incident Management Tool, to improve the utilization of incident investigation and analysis and, also, to create an event notification system for all employees. When an incident occurs, gathering as much information, as quickly as possible is the key to reducing recurrence. The Incident Management Tool after an incident initiation, generates automatic email notifications to managers, enabling them to provide everyone involved with key information on what actions need to be taken immediately. Thereafter, further action can be taken to reduce the risk of recurrence and change the accident and incident management process immediately by eliminating, reducing, isolating or controlling the hazard.
The Incident Management Tool, is a software platform, which involves every level of the organization, instilling a health and safety culture by integrating observation, incident management and prevention. Unlike paper based reporting and accident books, the Incident Management Tool is held the incident data in one place, accessible at any location which can be used to generate reports identifying incident trends.
The IMT (Incident Management Tool) is divided in 6 Steps:
Step 1: Initiation. In this step ANY employee initiates an incident report (near miss, work observation, injury) by filling out incident information and describe as much as possible (attach photos of the event – incident) and Incident Investigation workflow automatically is being assigned according to the incident nature followed by a notification mail sent to the assigned workflow.
Step 2: Root Cause Analysis, Actions. In this step, the Line Manager or Supervisor assigned (based on the area of responsibility), will investigate the root cause (i.e. using a ‘5 Why’ Root Cause analysis) and define actions by specifying the owners of actions and apply the deadlines. When actions have been specified, a notification email is being sent to all action owners involved.
Step 3: Root Cause Analysis and actions approval. In this step, the Root Cause Analysis and actions are being evaluated. When approved, all assigned parties would be notified, if rejected, goes back to step 2. Subject Matter Expert (SME) approves/rejects root cause or actions.
Step 4: Action Execution. In this step, the action owners describe the actions completed in details (attach photos).
Step 5: Action Verification. In this step, the actions are being verified and marked. Actions can be rejected and return to action owners for corrections or accepted and move to step 6.
Step 6: Incident Closure. In this step, all parties are being notified that this incident is closed. Subject Matter Expert (SME) verifies actions taken and closes the incident or rejects and go back to Step 5.
All types of incidents (injury, property and equipment damage, incidents with environmental impact) potential incidents, such as near-misses or observed unsafe acts or conditions are transparent between all employees in the organization improving communication, visibility and accountability.
