Purpose of the Program: To contribute to the improvement of the lives of vulnerable people and their family members, by providing social care and home nursing services of high quality. based on an advanced model with safe and effective person-centered care
Objectives of the program:
Creation of a Structure in Heraklion, Crete, which will provide at home combined services from a social worker, nurse, family assistant to vulnerable social groups. A total of 50 beneficiaries for a period of one year, throughout the prefecture of Heraklion, will receive services from specialized and well-trained staff.
΄΄Care at Home΄΄ supports the philosophy, which has been prevalent abroad for years now, of painless recovery and rehabilitation of the patient at home, for a faster and more effective recovery, due to the avoidance of hospital infections and unnecessary travel. It provides high-quality home care, for chronic or non-patients, for the elderly (partially self-supporting or bedridden), children and people with disabilities.
The specific action has no time duration, as long as there are funds available to support it. The needs of the beneficiaries are continuous.
Our organization is the only one in Crete that provides combined services at home, from four different specialties, and its ethics is based on the fact that each incident is first assessed by the social worker. The social worker is the one who will visit the beneficiary at home, will record and evaluate his needs, will draw up the social report and based on this, the services provided will be categorized with the other specialties. Furthermore, the social worker will be in contact with the person's immediate and wider family environment, in order to have a complete picture of the whole situation, to determine if there is abuse, domestic violence, neglect. It informs the beneficiaries about allowances and other social benefits to which they may be entitled, and helps them in the entire process required for drawing up a report file at KEPA.
Primary health care is provided by a nurse (Personal hygiene and cleanliness - vital signs measurement, urinary catheter placement, wound care, injection therapy, blood collection/transportation of blood and urine to the hospital, medication prescription)
The other specialty that provides services at home is the physical therapist assistant, who evaluates the motor functionality and balance of the person, putting them in a process of therapeutic exercise. Another specialty that provides home services is the family assistant, who deals with the payment of people's bills, their shopping and of course in combination the above specialties deal with accompanying people to their social events or leisure walks.
The staff fully cooperate and exchange views on case management, which is controlled by the social worker, so that there is the best desired outcome for their health, mental and physical, always with the guidance of their attending physician.
Also, an electronic file of the patient is kept at our institution, where he is informed of any changes in the evolution of his health, in accordance with the principle of preserving personal data.
Finally, in addition to specialized professional knowledge, the staff has the know-how of human contact, approach and communication, so that they can enter the beneficiary's home and consequently create a climate of cooperation and trust, very important elements for the development and evolution of our subsequent cooperation. The approach to the needs of the beneficiaries is also based on empathy, i.e. the ability to understand the individual's problem, but without ever identifying with it. The combination of all the above makes us unique, treating a beneficiary holistically, resulting in the best rehabilitation.

"Care at Home" is a Social Cooperative Enterprise based in Heraklion, Crete, established in 2017. We operate throughout the prefecture of Heraklion, in the city of Heraklion as well as in all remote villages.
COINSEP "Care at Home" is managed by an elected 5-member board of directors.
