AT-HOME CARE FOR RETIREES

Objective Action

The contribution to improving the quality of life for vulnerable individuals and their family members in the Heraklion prefecture of Crete. Our objectives:

  • 80% of the total 150 beneficiaries of the program live with dignity and improved functionality in their own familiar environment.
  • 80% of the total 150 beneficiaries of the program have avoided institutionalization and continue to be active members of their community.
 

Target Audience

In the Heraklion prefecture, there are officially 4,000 individuals with disabilities registered, from which, it is estimated that a significant percentage, around 30%, are facing serious illnesses (dementia, diabetes, cancer, psychiatric disorders). Moreover, they live alone in isolated regions of the inland areas, with low incomes, or even in conditions of extreme poverty.

 

 

Duration

The implementation of the program began in 2018 and continues up to the present day. This means that progress and work have been carried out from 2018 until now. However, the development of the program has not been completed yet and will continue for the next 2 years. This indicates that there are still tasks and goals to be achieved before the program is fully concluded.


Description

The employees of "Frontida sto Spiti" during its 5 years of operation have visited 1520 beneficiaries in the Heraklion prefecture, who fall within the aforementioned categories. The conclusion drawn from the conducted research is that 80% of them do not receive the required in-home support and assistance. Thus, they are not able to live with dignity and improved functionality within their own familiar environment, as active members of society, nor can they avoid institutionalization.

The proposed program supports the facilitation of high-quality services to vulnerable population groups, who seek relief and rest from the burdens of daily life. The main aim is to improve their lives and the lives of their family members in the Heraklion prefecture of Crete. The program offers social care and nursing services at home, based on an advanced model of secure and effective person-centered care.

Impact on Society

  • Improved functionality of all beneficiaries: 80%.

  • Improvement of mental health for 110 beneficiaries: 60%.

Initiative Location

Our action is currently underway in the Heraklion prefecture of Crete. Specifically, the action is being implemented in the capital of the prefecture, the city of Heraklion, as well as in 26 remote villages of the region. Our goal is to ensure that our initiative reaches these local communities and provides benefits and services to them. Depending on the nature of our initiative, the specific actions carried out in these areas may vary. We aim to enhance and improve infrastructure, services, and the quality of life for the residents of these areas.

Working with Organization

Our initiative is currently underway in the Heraklion prefecture of Crete. At this moment, there are no official collaborations taking place with other organizations, but we informally collaborate with local entities. Through these collaborations, we aim to identify the challenges faced by vulnerable groups in the area.

Alongside local partners, we gather information about the needs and challenges of the vulnerable groups mentioned. This helps us better understand the community's needs and adjust our actions accordingly.

Impacts on the Social Sector: The program contributes to strengthening social cohesion and justice by providing equitable access to care and support services for all population groups, regardless of their socioeconomic status or vulnerability.
